1. Who we are
Goldstar Care Solutions Pty Ltd (ABN 51 272 283 476) is a registered NDIS provider and an approved Support at Home aged care provider. We are committed to protecting your privacy and complying with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and relevant aged care and disability privacy obligations.
2. What we collect
We collect personal and sensitive information that is reasonably necessary to deliver care, including:
- Identification details — name, date of birth, address, contact details, next of kin.
- Health information — medical history, medications, treating practitioners, care plans, NDIS plan details, My Aged Care assessment information.
- Funding and government information — NDIS participant number, Centrelink/Medicare details, My Aged Care referral codes.
- Service delivery records — case notes, incident reports, communications with you and your representatives.
- Website data — pages visited, IP address, basic device information, where you submit forms.
3. How we use it
We use your information to plan and deliver safe, person-centred care; meet our regulatory and contractual obligations to the NDIS Quality and Safeguards Commission, the Aged Care Quality and Safety Commission, and other government agencies; communicate with you, your family, and your other care providers; bill funders and process payments; and improve our services.
4. Who we share it with
We share information only where necessary, including with: your treating practitioners and other care providers; the NDIS, My Aged Care, Services Australia, and other government agencies; auditors, insurers, and professional advisers under confidentiality; and IT service providers who help us run our systems. We do not sell your information.
5. How we protect it
We store records in access-controlled systems, restrict who can see what to a need-to-know basis, train every staff member on privacy obligations, and securely destroy records once retention obligations have ended.
6. Your rights
You can request access to or correction of your personal information at any time. You can also withdraw consent for non-essential uses, raise a privacy complaint with us, or escalate to the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
